Below you will find student forms which need completion for this school year.  Most of the forms are electronic and can be completed and submitted online.  Required forms must be completed prior to August 21, 2023.

Required Forms for All Students

1. Emergency Care Form

   • All students must have accurate emergency care information each school year. Parent/Legal Guardians update this information as well as all other Student/Family information in the SIS ParentVUE Online Verification/Update portal. 

     • Log into your SIS ParentVUE account 

2. Health Information Form - In SIS ParentVUE.

3. Digital Resource Consent - This parent consent allows your child to use FCPS approved digital tools that require parental consent. Directions on how to fill out the Digital Resource Consent Form can be found HERE. 

      4. Intent to Ride School Bus (Can be found in your ParentVUE account)

      5. After School Program Registration Form (ALL students must complete, regardless of intent to participate).  More information about our After School Program can be found below.
